Data Quality flags applied for the following categories of potential data entry errors:

  1. Physical Measurements – Completeness Checks

  3. Baseline Diagnostic Status (missing diagnoses within first 3 months) 

  4. Longitudinal Diagnostic Tracking (Patients Active >12 Months with no update in >12 months) 

  5. Medications – Incomplete Details

  6. Implausible or Out-of-Range Static Values

  7. Implausible or Inconsistent Longitudinal Changes

  8. Cross-Field Clinical Inconsistencies

  9. Misplaced or Reversed Measurements

  10. Inconsistent Dependency Errors

  11. Time-Based Contradictions

Priority groups are now the only reference point for missing data. As a result, this table has been removed:

Diagnosis Resolution Charts (National View Only)

  • 'Baseline-6m' bar removed

  • 'Outcomes at discharge' bar removed. Children previously resolved' in this bar (e.g. 2 marked resolved on hypertension chart) have been reassigned into the relevant time points on the cumulative 6/12/24m bars. (E.g. if discharged at month 9, should appear in the baseline-12month bar).

Data Completion - PCOS: 

Removed the PCOS option in the Data Completeness page in the SMURF for all male patients on the dashboard. As a result, there is no need to enter PCOS-related data for male patients — they will be automatically excluded from this option.

PCOS Monthly Data Completion Report: 

PCOS has been removed from left hand chart (patients who have been in the service for 12m or more) as PCOS does not resolve. As a result, this will no longer be tracked in data completion reporting here. 

EQ-5D-Y-5L:

  • Assessments are now located in the Data Completeness tab.

Priority Groups:

  • Patients who have a diagnosis code as excluded will retain this code in subsequent appointments and therefore will no longer display in patient priority groups. For example, if at initial appointment patient is marked as ‘excluded’ from Type 2 diabetes, all future appointments will be marked as ‘excluded’ for type 2 diabetes, unless actively changed by the clinic 

    • Patients active in CEW for >12 months, who have baseline diagnostic status (data in first 3 months) and no additional data entry for hypertension diagnostic status

    • Patients active in CEW for >12 months, who have baseline diagnostic status (data in first 3 months) and no additional data entry for type 2 diabetes diagnostic status

    • Patients active in CEW for >12 months, who have baseline diagnostic status (data in first 3 months) and no additional data entry for hypothyroidism diagnostic status

    • Patients active in CEW for >12 months, who have baseline diagnostic status (data in first 3 months) and no additional data entry for idiopathic intracranial hypertension diagnostic status

    • Patients active in CEW for >12 months, who have baseline diagnostic status (data in first 3 months) and no additional data entry for metabolic dysfunction-associated steatotic liver disease diagnostic status

    • Patients active in CEW for >12 months, who have baseline diagnostic status (data in first 3 months) and no additional data entry for obstructive sleep apnoea diagnostic status

Waist Circumference:

  • Dashboard now autocalculates waist:height circumference if the two measurements are entered within 1 week.

SMURF:

  • Text Update: “Gender” has been replaced with “Sex” (but continues to reflect biological sex; wording solely updated) 

Patient List:

  • Drug count column has been removed.

Results Pie Chart:

  • Will only show patients as "Not assessed/no data identified" if there was no other previous code entered. For example, if patient was coded as diagnosed but then coded with "Not assessed/no data identified" after, the pie chart will show them as Diagnosed.  .

Data Entry - Height

  • “No data related to height during this appointment due to physical difficulties or patient/carer declined assessment” is now an option when submitting patient height values.

Clinic Activity Tracker

  • Clinic Activity Tracker charts now start from the beginning of the month instead of from a previous data entry.

Hypertension:

  • Definition hover-overs updated.

T2DM Questionnaire:

  • Impaired fasting glucose removed from MDS list.

  • Impaired glucose tolerance removed.

  • Definition hover-overs updated.

MASLD:

  • Definition hover-overs updated.

IIH:

  • Definition hover-overs updated.

Hypothyroidism:

  • TSH result question removed from questionnaire.

  • Definition hover-overs updated.

Obstructive Sleep Apnea:

3 questionnaire questions removed:

  • Sleep study requested

  • Sleep study results

  • Is the child technology dependent?

PCOS:

  • “Not applicable (male)” category removed.

  • All data only relates to female patients.

  • Title Change: “PCOS (females only)”

MASLD:

  • Definition hover-overs updated.

Anxiety:

  • Definition hover-overs updated.

Depression:

  • Definition hover-overs updated.

ADHD:

  • Definition hover-overs updated.

Autism:

  • Definition hover-overs updated.

Binge Eating Disorder:

  • Definition hover-overs updated.

Education and Employment:

  • Definition hover-overs updated.

Clinic Appointment:

  • Definition hover-overs updated.

GLP-1s:

  • GLP-1 questions separated out by medicine instead of all in one form.

Discharge Point Prompt

  • At point of discharge, clinics can manually opt to text out any questionnaire, including those for PedsQL, to a patient via a form provided on the Specialist Review page.

  • Please note that all SMS questionnaires are sent to patients through manual selection in the Specialist Review page .
