Lipid Case Finding (Perfect)

View Priority Patients and Access Questionnaires

1. Once logged in the home page will be shown.

2. Select Your Healthier Futures Programmes.

3. Select the Lipid Detection & Optimisation Programme.

4. Click VIEW to enter one of the pathways on the dashboard.

  • Pathway 1 contains patients with established CVD:
    Age between 25 and 84 
    With either Ischaemic Heart Disease, Stroke or Peripheral Arterial Disease
    No Chronic Kidney Disease and Latest eGFR not below 30

  • Pathway 2 contains patients with CKD (chronic kidney disease) CKD and require primary prevention dose:
    Age between 25 and 84 
    Chronic Kidney Disease or eGFR < 60
    No Ischaemic Heart Disease, Stroke or Peripheral Arterial Disease

  • Pathway 3 contains patients with a QRISK3 >20% and require primary prevention dose
    Age between 25 and 84 
    QRISK3Score > 20
    No Ischaemic Heart Disease, Stroke, Peripheral Arterial Disease, Chronic Kidney Disease 
    Latest eGFR not below 60

5. Select Priority Patients to view the patient cohorts. For this programme, you can also find the patient engagement categories in this section.

6. Select VIEW to identify the patients within the priority group cohorts.

7. Click the column heading twice to order the patients with the highest recording.

8. Click the magnifying glass to view the SMURF review – individual patient information.

Eclipse Detect (Practice Patient Engagement)

Contact your patients via the preferred method of the practice. Eclipse Detect practices are required to engage with, educate and counsel patients for Your Healthier Future programmes themselves. If you would prefer that automated SMS campaigns are done on behalf of your location, you may sign up for Eclipse Complete (Eclipse Patient Engagement).

9. Once in a patient profile, you can view their medical information as well as any outstanding programme actions, including accessing a patient’s completed questionnaire or completing a questionnaire on behalf of a patient.

10. Patients will automatically move into the appropriate priority patient group as the clinical code enters the database, but this method will allow live tracking of patient review outcomes.
