NHS Health Checks Programme: Patient Engagement
View Priority Patients and Access Questionnaires
1. Once logged in the home page will be shown.
2. Select Your Healthier Futures Programmes.
3. Select the Health Check Programme.
4. Click VIEW to enter the Health Check dashboard.
5. Select Priority Patients to view the patient cohorts.
6. Select Select VIEW to identify the patients within the priority group cohorts.
Tip: You can use the Core20PLUS5 Filter to narrow down to criteria of interest within the cohort wherever the filter is available.
7. Click the column headings to sort patients by different criteria (ex. ACB Score)
8. Click the magnifying glass to view the SMURF review – individual patient information. You can use this page to access questionnaires a patient is eligible for and to fill out these questionnaires with a patient.
9. Patients will automatically move into the appropriate priority patient group as the clinical code enters the database, but this method will allow live tracking of patient review outcomes.
