Hypertension Optimisation (Perfect) Programme
View Priority Patients and Access Questionnaires
1. Once logged in the home page will be shown.
2. Select Your Healthier Futures Programmes.
3. Select the Hypertension Perfect Programme.
4. Click VIEW to enter the Hypertension Perfect Pathway dashboard.
5. Select Priority Patients to view the patient cohorts.
6. Select Select VIEW to identify the patients within the priority group cohorts.
7. Click the column headings twice to order the patients with the highest recordings if you would like to sort results by specific criteria.
8. Click the magnifying glass to view the SMURF review – individual patient information.
9. Patients will automatically move into the appropriate priority patient group as the clinical code enters the database, but this method will allow live tracking of patient review outcomes.
