View Patient Engagement and Questionnaire Responses

1. Navigate to https://secure.nhspathways.org/ (this will require a secure HSCN/N3 network)
2. Enter your username and password
You will have received your username via an email from the Eclipse support help desk
3. Select Sign In
4. Request your passcode via Email or SMS*
*By default the code will be sent to you NHS email, unless a phone number was provided at the later stage. You can also update your information within the system.
5. Check your Email (or Phone) for a 6-digit passcode.
6. Enter the passcode.
7. Select Enter Site.
8. Select Pneumococcal Protect
9. Enter the Pneumococcal Protect pathway by selecting “View”.
10. Select Patient Engagement to access the audit tracking for patient uptake to the programme (this contains the patient list of pending referrals).
11. This page walks through the engagement process to patients and the outcomes of those that have engaged.
What you can see within this table:
Identification: shows number of likely eligible patients
Patient Engagement: contains the lists of those that received the text, accessed the lander page, and completed the questionnaire.
Patient Participation: indicates the number of patients wanting the service.
Group 6a. Patients that would like a pneumococcal vaccine contains all the patients that have opted to receive a vaccine. These are further sorted into section 7 of the chart.
Action: Patient can be booked in for a vaccine appointment. Continue to step 12 for further instructions.Group 6b. Patients that have declined pneumococcal vaccine contains all the patients that have declined a vaccine in relation to this programme.
Action: Add SNOMED code “401086001” to a patient’s record to mark that the vaccine has been declined.
Group 6c. Patients that have not indicated whether they would like a pneumococcal vaccine contains all the patients that have not selected their preference regarding receiving a vaccine through this programme.
Group 6a is broken down into three further groups, under Patient Access:
7a. Patients able to attend their GP surgery - Patients who prefer to receive their vaccine at their GP surgery.
7b. Patients that would like a pneumococcal vaccine but report difficulty attending clinic - Patients who would like a vaccine, but may not be able to travel.
7c. Patients that would like pneumococcal vaccine but no access preference specified - Patients who have not set an attendance preference.
Groups 8 and 9 will populate in more detail following appointment booking.
Group 3b: Texts failed to send to patient is a group that may benefit from having their contact details updated. This category is for patients who do not have a phone number assigned to their record and therefore, cannot be reached via SMS for this programme.
14. Selecting the magnifying glass icon will open the patient’s portal (SMURF) to review the patient’s medical overview.
15. Once in a patient’s portal, access their completed questionnaire from “Review Last Qu” button – this allows for review of any cautions or accessibility issues.
You can also access a patient’s questionnaire by scrolling down to the Engagement History section.
16. You can book these patients for appointments in your chosen clinic manually by reviewing the Pneumococcal Protect pathway in a patient’s SMURF.
As the patient has their progress on their pneumococcal vaccination coded into the clinical record in accordance with their appointment status, they will move through the patient engagement flow to show in Patient Outcomes Groups
12. Manage your practice’s bookings for patients who agree to participate.
View the list of consenting patients via NHSPathways. Lists of consenting patients are sorted by preference in the Patient Engagement chart referenced earlier in this guide.
A member of the practice team contacts the patient directly to book the vaccination appointment
13. Selecting “view” will open the patient grid.
You can use the Core20PLUS5 Filter at the top of the table to further narrow down results and tailor your desired patient list.
