
Your Healthier Future - NHS Health Checks Programme
1. Navigate to https://secure.nhspathways.org/ (this will require a secure HSCN/N3 network)
2. Enter your username and password
You will have received your username via an email from the Eclipse support help desk
3. Select Sign In
4. Request your passcode via Email or SMS*
*By default the code will be sent to you NHS email, unless a phone number was provided at the later stage. You can also update your information within the system.
5. Check your Email (or Phone) for a 6-digit passcode.
6. Enter the passcode.
7. Select Enter Site.
8. Select Your Healthier Future Programmes.
9. Find the NHS Health Check pathway and Select “View”.
10. Select Patient Engagement to access the audit tracking for patient uptake to the programme (this contains the patient list of pending referrals).
11. This page walks through the engagement process to patients and the outcomes of those that have engaged.
What you can see within this table:
Identification: shows number of likely eligible patients
Patient Engagement: contains the lists of those that received the text, accessed the lander page, and completed the questionnaire.
Patient Participation: indicates the number of patients wanting the service.
Group 6a. Patients that would like to receive the service contains all the patients that have opted for the Health Check.
This is broken down into two further groups, under Patient Outcomes:
7a. Patients that would like to receive service and have no barriers to access - patients for which no accessibility flags have been raised on their questionnaire. Patients can have referral made without contact
7b. Patients that would like to receive the service but have potential barriers to access - patients may have accessibility issues with the programme, require questionnaire review followed by potential patient contact and alternative advice if not able to access the programme. (e.g. wi-fi accessibility issues)
12. Selecting “view” will open the patient grid.
The patient grid:
Selecting the magnifying glass icon will open the patient’s portal (SMURF) to review the patient’s medical overview.
Review column will display the review outcome that has been applied to the patient’s portal (e.g.
Referred to SMR Medication Review).Last Patient Questionnaire provides quick access to the patient’s DMWP invitation questionnaire response.
Patient identifiable information is available when logged into secure.nhspathways.org, with role-based access.
Core20PLUS5 Filters can be applied to the patient list.
At the top right of the grid you will find an export to excel feature, if this is a preferred view of your patient list.
13. You can now view the patient portal:
14. Once in a patient’s portal, access their completed questionnaire from “Review Last Qu” button – this allows for review of any cautions or accessibility issues.
15. You can now view the DWMP Questionnaire
Complete the Standard e-RS Referral
a. Access the e-Referral System (e-RS). Referrals can only be made via EMIS, SystmOne or Vision.
b. Download the Referral Template. Templates are available on the NHS Digital Weight Management Programme GP Template Site (practice ODS code required for access).
c. Use the Template Electronically. The templates are designed for digital use only and should not be printed or re-scanned.
d. Practices will be entitled to £11.50 per referral into the Digital Weight Management programme. Only one referral per patient may be claimed under this ES.
e. Practices must make a manual claim to their commissioner for payment. Claims must be on the basis of the number of unique patients with NHS Digital Weight Management Programme: SNOMED code is 1402911000000108 – Referral to National Health Service Digital Weight Management Programme (procedure)
16. Once a referral has been made on the e-Referral System, select “review” and document within Eclipse that this referral has taken place.
If a patient is not suitable for referral, select the appropriate option.
17. This review outcome will be shown under “DWMP Pathway Review” on the patient list.
18. Once the SNOMED code for the referral has been picked up within the Eclipse system (the pathway will rebuild each Monday), the patients will appear in group 7c on the patient engagement flow.
