Lipid Case Finding: Patient Engagement

1. Navigate to https://secure.nhspathways.org/ (this will require a secure HSCN/N3 network)

2. Enter your username and password 

You will have received your username via an email from the Eclipse support help desk 

3. Select Sign In

4. Request your passcode via Email or SMS

*By default the code will be sent to you NHS email, unless a phone number was provided at the later stage. You can also update your information within the system.

5. Check your Email (or Phone) for a 6-digit passcode.

6. Enter the passcode.

7. Select Enter Site.

8. Select Your Healthier Future Programmes.

10. Find the Lipid Detect and Optimisation pathway and Select “View”.

  • Pathway 1 contains patients with established cardiovascular disease

  • Pathway 2 contains patients with CKD (chronic kidney disease)

  • Pathway 3 contains patients with a QRISK3 >10%

  • Pathway 4 contains patients with Type 1 Diabetes

11. Select Priority Patients to access the audit tracking for patient uptake to the programme (this contains the patient list of pending referrals).

12. This page walks through the engagement process to patients and the outcomes of those that have engaged.

Eclipse Detect (Practice Patient Engagement)

Contact your patients via the preferred method of the practice. Eclipse Detect practices are required to engage with, educate and counsel patients for Your Healthier Future programmes themselves. If you would prefer that automated SMS campaigns are done on behalf of your location, you may sign up for Eclipse Complete (Eclipse Patient Engagement).

What you can see within this table:

Identification: shows number of likely eligible patients

Patient Engagement: contains the lists of those that received the text, accessed the lander page, and completed the questionnaire.

Patient Participation: indicates the number of patients wanting the service.

  • Group 6a. Patients that would like to receive the service with no contraindications and no statins prescribed 4 weeks after completing questionnaire contains all the patients that have opted for the Lipid Case Finding programme, are likely candidates for statins, and have been awaiting review the longest.

    Action: Prescriber to issue statin prescription. Higher priority than 6b.

  • Group 6b. Patients that would like to start a statin, have no contraindications identified less than 4 weeks since completing their questionnaire contains all the patients that have recently opted for the Lipid Case Finding programme, and are likely candidates for statins.

    Action: Prescriber to issue statin prescription.

  • Group 6d. Patients that would like to receive the service but may be interested in an alternative medicine contains all the patients that have recently opted for the Lipid Case Finding programme, and would like to try a treatment other than statins.

    Action: HCA/nurse to contact patient to further discuss prescription of lipid lowering therapy.

  • Group 6e. Patients that have declined a statin but do not have it coded in their record contains all the patients that have declined statin use, but need their record updated.

    Action: Practice admin to code patient as “statin declined”.

  • Group 6f. Patients with a contraindication on their record and require a review contains all the patients who are interested in statins, but need further review before receiving a prescription.

    Action: HCA/nurse to code into clinical record.

  • Group 6g. Patients that require an up-to-date liver function test prior to receiving a statin prescription contains all patients who require LFTs prior to receiving a statin.

This is broken down into three further groups, under Patient Outcomes:

  • 7a. Patients who started a statin >= 3 months ago but have not received a LFT since - patients who have been prescribed statins, but require an LFT.

  • 7b. Patients now on a statin with up-to-date liver monitoring - patients who have been prescribed statins

  • 7c. Patients newly coded as statin declined - Patients who have declined statins based on patient engagement efforts.

Once patients have been started on a statin, they will appear in group 7 of the priority patient list. After 3 months, patients without a repeat blood test will be highlighted and can be actioned by the practice.

Patients not contacted:

Remaining listed priority patient groups consist of those that did not meet the low-risk eligibility criteria (e.g. no baseline liver function tests). These patients can be reviewed for potential lipid lowering therapy at the clinical discretion of the practice.

Group 3b: Texts failed to send to patient is a group that may benefit from having their contact details updated if your practice has signed up for Eclipse Complete (Eclipse Practice Engagement) to run questionnaire campaigns on behalf of your practice.

13. Selecting “view” will open the patient grid.

The patient grid:

  1. Selecting the magnifying glass icon will open the patient’s portal (SMURF) to review the patient’s medical overview.

  2. Review column will display the review outcome that has been applied to the patient’s portal (e.g.
    Referred to SMR Medication Review).

  3. Last Patient Questionnaire provides quick access to the patient’s Your Healthier Future invitation questionnaire response.

    Patient identifiable information is available when logged into secure.nhspathways.org, with role-based access.

  4. Core20PLUS5 Filters can be applied to the patient list.

    At the top right of the grid you will find an export to excel feature, if this is a preferred view of your patient list.

14. Once in a patient’s portal, access their completed questionnaire from “Review Last Qu” button – this allows for review of any cautions or accessibility issues.

15. You can document patient reviews in the patient portal by clicking “Review” and applying the outcome of the patient review. This will highlight the patient contact within the patient list to avoid re-contacting your patients. This will also allow the targeted programme activity to be tracked for your practice, within Eclipse.
