Practice Patient Engagement (Eclipse Detect) service:

If your practice has signed up for the Practice Patient Engagement module for a Your Healthier Future programme, the service will:

• Identify and prioritise patients aged 18 and over, with a clinical indication for inclusion in that programme.

Eclipse Patient Engagement (Eclipse Complete) service:

If your practice has signed up for the Eclipse Patient Engagement module for Your Healthier Future programme, the service will:

• Identify and prioritise patients aged 18 and over, with a clinical indication for inclusion in that programme

• Engage with eligible patients to educate, counsel and consent them for starting the programme. This is done via an online questionnaire that is accessible via Eclipse.

• Facilitate efficient practice reviews of consented patients to initiate treatment within the programme.

The solution being offered for Eclipse Patient Engagement utilises Eclipse’s digital patient engagement platform, which will automatically identify your eligible patients, engage with them via SMS, educate and counsel them using a lander page and consent them to be initiated on that programme’s treatment method.

Where pro forma permissions have been granted, the CQC-accredited Putting Patients First patient engagement team will be contacting patients on behalf of the practice to increase patient uptake to the programme.

This patient support programme is part of the Cambridgeshire & Peterborough Your Healthier Future programme, clinically led by Dr. Jessica Randall-Carrick (Co-Lead for Cardiovascular Disease Prevention in Cambridgeshire & Peterborough ICS).

This programme will contribute to achieving QOF targets for practices, and there will be some additional funding for participating practices.
