Practice Pro-Forma

1. Overview & Benefits 
Part of the Your Healthier Future initiative, this programme helps practices update patient health records (e.g. weight, lifestyle, ethnicity) in a respectful, consent-based way. 

Benefits include: 

  • Identifying patients needing extra support 

  • Improving cardiovascular and metabolic outcomes 

  • Supporting personalised care without judgement 

  • Financial incentives for practices 

 

2. Participation Options 
Choose how your practice takes part: 

1. Practice-Led Engagement 

  • You contact patients directly and manage follow-up 

  • Full control; £1,000 payment if targets met 

2. Centralised (Eclipse) Engagement 

  • Eclipse team contacts patients via SMS/phone 

  • You review returned health info 

  • Lower admin burden; £400 payment 

3. No Participation 

  • No action needed; no payment 

All communications are consent-based, confidential, and non-stigmatising. 

 

3. About the Programme 
Focuses on updating key health data (e.g. weight, smoking, alcohol, ethnicity, language). 

  • Aims to support overall wellbeing, not judge 

  • Conversations are handled with care and respect 

  • Reduces health inequalities 

 

4. How Patient Engagement Works 

  • Practice-led: You choose patients and how to engage them 

  • Centralised: 

  • Eclipse identifies eligible patients 

  • Patients are contacted centrally for consent and data update 

  • Information is coded into your system with permission 

  • Summary reports provided to support next steps 

 

5. Confidentiality & Data Protection 

  • All data is secure, consented, and anonymised in reports 

  • Evaluation includes patient feedback and outcomes 

  • GDPR-compliant, with full DPIA and DSA available 

  • Data controller: C&P ICB | Processor: Eclipse (Prescribing Services Ltd)
