We invite your practice to join the Growth Accelerator Programme, a patient engagement and activation initiative designed to support individuals with MSK or mental health conditions who are at risk of work loss or continued health decline.
This programme leverages the Eclipse platform to identify, stratify, and systematically engage patients aged 16–67 with repeated fit note use or mental health/physical health needs, enabling referral to local coaching and support services via the NECs Hub.
Programme Overview
Objective:
To engage and activate eligible patients with MSK or mental health conditions—especially those with repeat fit notes—into timely support, coaching, and onward referral pathways that optimise health, wellbeing, and working potential.
Key Components of the Programme
Identification of Eligible Patients:
Patients aged 16–67 meeting SY ICB Growth Accelerator criteria will be identified via Eclipse based on MSK or mental health conditions and fit note history.
Digital and Telephone Engagement:
SMS invitations will direct patients to secure portals to complete a questionnaire, register interest, and provide insight on support needs. Non-responders may be contacted by the NECs Hub.
Coaching Hub Referral:
Patients can be referred to the NECs local coaching Hub for further engagement, activation, and personalised support.
Data Reporting:
Programme requires monthly patient-level reporting. This will include the secure transfer of NHS Numbers of participating patients to NHSE for evaluation and service optimisation.
