Programme Overview 

The GLP-1 Weight Management Support Programme is designed to help practices identify and engage patients who may benefit from GLP-1 medication for weight management. This initiative uses Eclipse searches, drawing on data from your core clinical system (such as EMIS or TPP), and digital questionnaires to stratify patients who are likely to meet national eligibility criteria for GLP-1 support. 

The programme aims to: 

  • Proactively identify adults with obesity or high BMI who could benefit from GLP-1 medication, in line with national guidance. 

  • Educate and inform eligible patients about weight loss medication options before any GP review, using digital outreach and a virtual support team. 

  • Reduce practice workload by ensuring only those most in need and eligible are invited for clinical review. 

  • Support improved health outcomes for patients living with obesity, while ensuring safe and equitable access to new treatments. 

Eligible patients will first receive digital information via SMS, including a secure link to an educational landing page. This page will provide clear information about GLP-1 medication and guide patients to their personal online portal, where they can complete a short questionnaire to share their preferences and provide additional information. Patients who are contacted by the Putting Patients First virtual support team by telephone will receive the same education and complete the questionnaire during the call. 

All questionnaire responses are securely integrated into the programme pathway, enabling accurate patient segmentation and ensuring your practice can efficiently review and manage those most likely to benefit. Your practice will be notified before any patient communication begins, and you will receive step-by-step guidance on accessing patient lists and documenting reviews. 

Key outcomes include increased patient engagement, improved documentation of risk and eligibility, and streamlined referrals for GLP-1 support.
