Practice Onboarding 
Phase 2 practices are contacted and invited to join the enhanced programme via email, with a proforma to confirm participation, identify key contacts, and outline implementation preferences. 

Patient Engagement 
Eligible patients are digitally invited to participate. They receive access to a personalised online report highlighting outstanding diabetes care elements, including key monitoring, vaccinations, lifestyle inputs, and opportunities for treatment optimisation. 

Patient Self-Activation 
Through the portal, patients can review their care needs, request more information, indicate readiness to take action and book a support call with our review team. 

Support and Handover 
Patients are advised to follow up with their GP practice to implement agreed actions. With the patient’s consent, individual reports are securely shared with the practice and uploaded to patient records. 

Practice Enablement and Tracking 
Practices also receive a summary grid of outstanding actions and are supported to implement key items, with ongoing tracking of progress and outcome completion. 

Feedback and Evaluation 
Feedback is collected from both patients and practices to inform continuous improvement, alongside outcome data to support programme evaluation and scale-up.
