NNUH Familial Hypercholesterolaemia (FH) Genomic Project - Practice Pro-Forma A new Familial Hypercholesterolaemia identification service is being run in partnership with general practice, the Norfolk and Norwich University NHS Trust and the NHS Eastern Genomic Medicine Service Alliance (GMSA). We are working with healthcare professionals across Norfolk & Waveney to help educate and, where appropriate, treat people with high blood cholesterol. The aim of this project is to find and treat FH as early as possible.Your eligible patients (adults with a total cholesterol greater than 7.5mmol/L or LDL-C greater than 4.9mmol/L). will be sent a text with a link to the following information zone from which they can access the patient portal to complete a questionnaire where their history and personal preferences regarding this project can be registered. https://www.nhsconnect.org/Projectinfo/cholesterol-optimisation-fh-norfolk.html Patients identified as being at-risk of FH will be invited for a screening appointment with an FH Clinical Nurse Specialist and referred for genomic testing if the screening indicates this need.
