Project overview:
An exciting new Type 1 Diabetes initiative is being rolled out locally. ECLIPSE has been asked by the RUH Bath Diabetes team to build an integrated data service. This would initially enable a subset of our mutual patients living with T1DM to be more accurately monitored, followed-up, and alerted about their condition by the secondary care team. This unified diabetes dashboard would seek to include linked telemetry data (Continuous Glucose Monitors (CGM) and Hybrid Closed Loop systems (HCL)) for fully consented patients and will enable the specialist teams to understand where urgent care reviews are necessary, alterations to medication regimes should take place, and to stratify recall lists by clinical need. The benefit of such a service is hoped to be better monitoring, smarter allocation of care resource, and greater insight about vulnerable and ‘high-risk’ patients.
Our only ask of you is to prescribe Dexcom One+ CGM sensors:
As part of this project we will be asking patients to move onto Dexcom One+ Continuous Glucose Monitoring (CGM) sensors, as this is currently the only device compatible with the monitoring system. We will contact the patients directly to offer and explain the switch from other CGM devices and we will notify you if they plan to make the change.
Information Governance:
N.B. - No patient identifiable data will be shared for any patient who does not explicitly consent to enrolment in the programme (via either our online engagement process or telephone service), and consent to ongoing involvement can be withdrawn at any time.
