Project overview:
A new digital programme is being offered to practices across Lincolnshire to support the delivery of Structured Medication Reviews (SMRs) to patients at the highest risk of medicines-related harm. This service aims to improve outcomes and reduce risk for patients on multiple medications or with complex long-term conditions.
The initiative will be powered through Eclipse, which will centrally identify, engage, and support patients to complete an SMR using digital-first contact and self-referral tools. The approach prioritises personalised care and makes it easier for both patients and practices to take part.
Objectives
To ensure patients at risk of medicines-related harm are proactively reviewed.
To support polypharmacy and long-term condition management in line with national SMR guidance.
To reduce inequalities by identifying and engaging patients from underserved or deprived populations.
To support practice teams in optimising workflow and capacity through centralised patient engagement.
To create a repeatable, scalable model for digital-first SMR delivery across ICBs.
1. Patient Identification:
Patients eligible for SMR will be identified based on national guidance and include those:
in care homes;
with learning disabilities;
with complex and problematic polypharmacy, specifically those on 10 or more medications;
on medicines commonly associated with medication errors and risk of harm;
with severe frailty who are particularly isolated or housebound or who have had recent hospital admissions and/or falls;
using one or more medicines associated with dependence or withdrawal symptoms, from the following groups: antidepressants; opioids; gabapentinoids; benzodiazepines; and Z-drugs.
Eligible patients will be identified centrally using Eclipse’s analytics platform - and those with an SMR risk score >10 and on >10 medications will be prioritised for engagement.
2. Patient Engagement and Prioritisation:
Identified patients will be contacted through Eclipse’s secure digital platform.
SMS invitations will direct patients to an informative landing page.
Patients will complete a short online form to share medication experiences, concerns, and preferences.
Those expressing interest will be triaged and prioritised for an SMR appointment.
The process is fully integrated with EMIS/SystmOne and all responses will be shared with the practice.
3. Structured Medical Review:
Once the patient completes the form and opts in, they will be offered an online booking option for a Structured Medication Review with a pharmacist or clinical lead at your practice.
Appointments can be managed via standard clinic templates.
Practices will receive full support in configuring booking links, viewing patient submissions, and documenting outcomes.
A feedback questionnaire will be sent post-review to help evaluate programme impact.
