Health Check Programme

The Suffolk NHS Health Check+ programme has now been activated across your region, offering eligible patients the opportunity to receive their enhanced health check. The pilot is being delivered by the Suffolk GP Federation, who are responsible for providing the service across participating areas.
Activation of this programme will commence a bespoke Health Check Pathway within Eclipse. This will provide a list of all eligible patients, alongside additional priority groups, which can be further filtered and prioritised by parameters such as levels of deprivation, ethnicity, comorbidities, and cardiovascular risk factors. Inclusion is based on being within the programme’s set age range and not already diagnosed or treated for specific conditions.
Exclusion criteria include those with pre-existing diagnoses or active management of:
Coronary heart disease
Chronic kidney disease (stage 3–5)
Diabetes
Hypertension
Atrial fibrillation
Transient ischaemic attack
Familial hypercholesterolaemia
Heart failure
Peripheral arterial disease
Stroke
Current statin prescription for cholesterol-lowering
A documented 10-year cardiovascular disease risk of 20 % or higher from any prior check
Severe mental illness (SMI) – covered under annual SMI physical health checks
Learning disability (LD) – covered under annual LD health checks
Those meeting these exclusion criteria are presumed to be already under clinical management or addressed through other structured health check programmes and are not part of the routine NHS Health Check recall cycle.
Where permissions have been provided, prioritised patients will be contacted via the Eclipse platform, which will send an SMS to the patients, with a link to a lander page and questionnaire, which will inform, educate, and invite patients to attend a health check via an online secure questionnaire. This will capture patient consent and preferences within an Eclipse Health Check dashboard.
