The Suffolk NHS Health Checks programme has been activated across your region, providing your patients with the opportunity to attend their free NHS Health Check and express preferences in relation to this. Participating NHS Health Check Providers in Suffolk and Waveney GP practices and pharmacies will receive remuneration for delivering the NHS Health Check. This is being implemented by Suffolk GP Federation.

Activation of this programme will commence the Health Check Pathway within Eclipse. This will provide a list of all eligible patients, alongside additional priority groups, which can be further filtered and prioritised by parameters such as levels of deprivation, ethnicity, comorbidities, and risk. SMI and LD patients are excluded from this list, and will be invited by practices for SMI/LD checks – if full components of an NHS Health Check are also done as part of this, they can be coded and will count towards the NHS Health Check target.  

Where permissions have been provided, prioritised patients will be contacted via the Eclipse platform, which will send an SMS to the patients, with a link to a lander page and questionnaire, which will inform, educate, and invite patients to attend a health check via an online secure questionnaire. This will capture patient consent and preferences within an Eclipse Health Check dashboard.
